Please print and use this form to ask for donations.  You will need to meet your $350.00 (plus your $50.00 Membership fee) to attend the week.  Using the Solicitation Letter is an easy way to ask family, friends, or even your dentist or doctor to donate to this great cause.  Each donation you receive will be credited toward your fees for the week.  If you bring in more that your needed fees, it will help provide more materials for the great work we are doing.
